SPBT Authorization Form (Exchange-based Parts System)

NOTE: Exchanges will only be accepted with freight paid, the material must be in perfect condition to be reused and reconditioned, if not under these conditions, the value of the damaged parts will be billed. This form must be attached to the properly completed invoice.

I declare that I am aware that if I have not returned the exchange of the acquired item within 7 business days, I authorize the billing of the same value of the exchange-based part

This form is valid for 30 days from the date of issue.
